New Patients

Here at Sween Dental we are excited to welcome you! Please see below for our new patient forms below that you can print and complete at home and bring with you to your first appointment. If you are unable to do so, arrive 15 minutes early for your appointment and complete them in office.

First Appointment (Adult)

At your first appointment, you will meet with Dr. Sween, at which time, we will take all necessary X-rays. We will then do a thorough exam of your teeth, gums, and do an oral cancer screening. We will appoint you with one of our hygienists for the proper cleaning of your teeth, recommended by Dr. Sween

Children’s Appointment (Ages 3-11)

We try to make going to the dentist a fun experience! Your child will be scheduled with one of our friendly hygienists who will perform a prophylaxis (cleaning), which will include a fun segment of counting teeth to see how many they have or if they have any loose ones (mainly for the younger ones). An exam by Dr. Mark is filled with fun analogies to teach young minds. We will then assess them for any necessary X-rays and fluoride treatments at that time.
